A course designed to enhance the skills of the supervisor and/or investigation co-ordinator to optimize data collection for the investigation of lower level events.

2-Day Lead Investigator Skills Extension Course (16 hours)
Designed to assist personnel who will typically lead investigations to provide them with real and practical skills for conducting investigations.

• Organisational Accidents and the error chain

About ICAM
The Incident Cause Analysis Methodology (ICAM) was developed by BHPBilliton in 2000/01. John Placanica was a member of the development team which comprised BHPB employees and external Human Factors consultants together with James Reason (the developer of the Reason Model of accident causation).
The ICAM training course and materials used by Placanica & Company Pty Limited are based on the Reason Model and the incident cause analysis methodologies as developed by BHPB.
ICAM is not the subject of an exclusive licence, and the training and materials as developed by BHPB are freely available to any end-user and training service provider.
4-Day Master Class for Lead Investigators
(34 hours)
A course designed specifically for investigators leading level 3 and 4 investigations to ensure that a rigorous QA/QC process is applied on site and prior to investigation outcomes being delivered to the company executive.
